Pregnancy in pulmonary arterial hypertension (PAH) is associated with substantial risks. Endothelin receptor antagonists (ERAs), including macitentan and bosentan, are contraindicated in pregnancy due to teratogenic effects observed in animal studies.

Long-term intravenous (IV) epoprostenol is a key therapy for pulmonary arterial hypertension (PAH); however, rare immune-mediated conditions occurring during therapy remain incompletely characterised. 

Early graft dysfunction following lung transplantation is commonly attributed to primary graft dysfunction (PGD). However, other rare etiologies may present with similar clinical and radiologic features. We report a unique case of a 58-year-old male with idiopathic pulmonary fibrosis who underwent bilateral lung transplantation.
