Global PH Consult: Case #1

The first Global PH Consult case discussion webinar bringing together a clinician and PH specialist to explore a real-world anonymised complex PH case. Global PH Consult is a PVRI Access to Care Workstream initiative that creates opportunities for clinicians to discuss challenging PH cases, hear different specialist perspectives, and contribute to shared learning across the global PH community.
Case overview
A 37-year-old female with a history of atrial septal defect closure in 2019, well-controlled hyperthyroidism, and hepatitis B presented in early 2024 with relapsed hyperthyroidism and, for the first time, severe pulmonary arterial hypertension (PAH) diagnosed at our heart center. Her condition was assessed as WHO Functional Class III, with elevated NT-proBNP (3600 pg/ml) and significant pulmonary artery pressures (estimated sPAP 100 mmHg), an enlarged RV, and moderate tricuspid regurgitation. By January and July 2026, she developed worsening edema, hypoalbuminemia (albumin ~ 21 g/L), polyserosal effusion, easy fainting (low BP), and low spO2, requiring hospitalizations for albumin replacement and short-term intravenous iloprost infusions, while on a maintenance regimen of bosentan 125 mg BID, tadalafil 20 mg QD, furosemide dosed based on her fluid retention, spironolactone 50 mg, and bisoprolol 2.5 mg. Clinical improvement has been limited. She has not undergone right heart catheterization since 2024. Risk stratification indicated high risk via the COMPERA 2.0 score.

Our case submitter
Minh Ly Nguyen graduated from Hanoi Medical University in 2008 and earned her Ph.D. in Medicine in 2020. She is currently a lecturer in the Department of Cardiology at Hanoi Medical University and concurrently serves as the Deputy Director of the Cardiovascular Center at Hanoi Medical University Hospital. Her areas of deep expertise include congenital heart disease, pulmonary arterial hypertension (PAH), and Doppler echocardiography. With nearly 20 years of education, research, and clinical experience in treating pulmonary arterial hypertension—specifically in patients with congenital heart disease, CTD associated PAH, advanced idiopathic PAH, and pregnant women with PAH—Dr. Ly has significantly contributed to advancing the management and care of PAH patients in Vietnam
