Abstract

In March 2025, reflecting on a career spanning more than four decades, I was asked to recount a pivotal moment from my early career that got me interested in pulmonary vascular disease.

During my second month as an intern in 1977, a young man in his mid-20s was admitted to me in the Medical Intensive Care Unit (MICU) from a state penitentiary. He was in acute respiratory failure from pulmonary edema and had pulmonary hypertension, confirmed using a Swan-Ganz catheter - a procedure standard in the MICU at the time for measuring cardiopulmonary hemodynamics in many patients in acute respiratory failure.
